Any party interested in submitting a Prequalification Questionnaire in response to the posted advertisement should download, complete and return the attached form to the address included therein prior to the stated deadline. Parties that have not completed the fillable form (click "View") to retrieve the Prequalification Questionnaire will not be provided notice of University changes to the qualification process, schedule, etc.

PREQUALIFICATION PROCEDURE Provide at least five (5) project references on the attached Project Data Sheet (Exhibit A). Only Cabinet, Millwork & Finish Carpentry subcontractors passing the prequalification process will be allowed to participate in the bidding process for the above referenced projects. Any potential subcontractor that is not pre-qualified by the University will be excluded from submitting a bid to a qualified Bidder (i.e., General Contractor) for the above referenced projects.
